Licensed Lead Care Manager – CenCal Health (San Luis Obispo & Santa Barbara Counties)

CenCal Health is seeking a Licensed Lead Care Manager to support the Enhanced Care Management (ECM) initiative under California’s CalAIM program. This vital role coordinates comprehensive, person-centered care for high-need Medi-Cal members across San Luis Obispo and Santa Barbara Counties. The Lead Care Manager ensures continuity of care, supports staff, and leads a care team in compliance with CenCal and DHCS standards.

Location: San Luis Obispo and Santa Barbara Counties, CA

Job Type: Full-Time | Field-Based & Remote Hybrid

Key Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans for ECM-eligible Medi-Cal members.
  • Coordinate services across medical, behavioral, dental, and social domains, ensuring whole-person care.
  • Lead, train, and mentor care management staff, supporting the successful delivery of CalAIM initiatives.
  • Perform outreach and engagement efforts with high-need populations, including field-based visits.
  • Document member activities and progress in compliance with CenCal Health and DHCS protocols.
  • Facilitate interdisciplinary team meetings and collaborate with community-based organizations and providers.
  • Ensure quality performance standards are met and reported

  • Maintain thorough knowledge of ECM workflows, community resources, and Medi-Cal benefits.
  • Minimum Qualifications:

  • Active California licensure as an LCSW, LMFT, or LPCC.
  • Bachelor’s degree in nursing, social work, or related healthcare field (Master’s preferred).
  • Minimum 2 years’ experience in case management or care coordination, especially with high-risk Medi-Cal populations.
  • Demonstrated knowledge of CalAIM, ECM, and managed care operations.
