POSITION SUMMARY:
Responsible for supervision and training of individuals with developmental disabilities. Works closely with Case Manager & Program Manager to assist individuals in the development of their skills to their full potential, and foster their independence in a positive and caring manner. Completes required documentation.
DUTIES AND RESPONSIBILITIES:
- Use the established methodology for achieving client goals and track progress.
- Provides care, supervision, instruction and/or job coaching for the individuals we serve, services might be provided in person, remotely or in the community.
- Participates in weekly online videos.
- Development of activities, curriculum and schedules for programming in person or remotely.
- Assists the Case Manager in implementation of the Individual Service Plans (ISP's)/Individual Habilitation Component (IHC) and provides documentation as directed by the Program Manager.
- Assists with personal hygiene, toileting, feeding, and ambulation needs as needed.
- Accurately and legibly evaluates and documents the individuals’ progress through daily charting and log notes in client electronic file.
JOB REQUIREMENTS:
- High school diploma or general education degree (GED). Prefer three to six months of related experience working with developmentally disabled individuals or other special populations.
- Must possess valid California Drivers License. Requires ability to pass background check, physical, Tb and drug test.
- Must be able to push and pull up to 50 pounds.
- Must be able to use computer for daily notes, creation of YouTube videos and video conference.
